Terms and Conditions

• This promotion is only available within the United States: Only Fryers installed in the United States and End-Users located within the United States are eligible for this promotion.
• Rebates will be paid in the form of a pre-paid promotional debit card (the “Card”).  Card values are in US dollars.
• Limited to one rebate per serial number.  This offer cannot be combined with any other discounts or offers. 
• Fryer must be invoiced between Dec 1 and February 28, 2018 to be eligible for this promotion.
• Dealers, reps, service agents or other middle men cannot participate and cannot register to receive Cards on behalf of End-User Customers. Welbilt reserves the right to deny issuance of any Card to parties other than the End User Customer.

• Submissions for rebate must be received at the landing site no later than April 30, 2018.  
• Information required to receive the Card includes name, address, serial number, copy of invoice, and proof of installation or start up.  Invoice and proof of installation can be uploaded at the landing page.
• Please allow 4 weeks to receive your Card after you submit your information.  Once received, Cards must be activated prior to use according to the instruction received with the Card.  If you do not receive your Card, please contact fsmaretking@welbilt.com. All Cards are subject to, and must be redeemed in accordance with,  OmniCard’s “Cardholder Terms and Conditions”
• Data collected as part of this program can be used for marketing purposes by Welbilt and its affiliates, but will not be sold or shared with other unaffiliated entities.
• The value of the Card may be deducted from any funds expected if the fryer is returned.
• Any participant that requests fulfillment represents to Welbilt that they are receiving the rebate on behalf of their company or employer and that the actions of the employee abide by their company’s gift policy.
• Welbilt does not give out tax advice – recipients should consult their tax advisor
